This program is for Sunday, April 13, 2025 for a Kindergartner-4th grader basketball player from 5:00-6:00 PM. There are NO REFUNDS for this program once registered. Injury and Liability Release Form
In consideration for letting your child participate at Sunday Skills Sessions with the Thomas'
I hereby release Deon and Emily Thomas and Redeemer Lutheran Church of Park Ridge, its trustees, employees, agents and representatives from any and all liability and agree to not raise any claims or institute any legal action against the Thomas' or Thomas Training, its trustees, employees, agents or representatives based upon any cause of action in my favor that arises out of or in connection with my child's participation in Sunday Skill Sessions with the Thomas'. This release shall apply to any loss of or damage to my property, and to any personal injury (including death) that I suffer, including, without being limited to, any loss, damage or injury sustained or allegedly sustained by me due to the negligent acts or omissions of the trustees, employees, agents or representatives of Thomas Training and Sunday Skill Sessions with the Thomas'.
I fully understand and assume all of the risks, dangers and responsibilities connected with basketball and Sunday Skill Sessions at Redeemer Lutheran Church in Park Ridge. By filling our the registration form and paying for the program, I am agreeing with the above waiver.
