Sunday, August 10, 4th-8th Grade class. We encourage parents to stay and watch or you are more than welcome drop and pick back up. Class runs from 5:00-6:00 PM in Park Ridge at 1006 Gillick St, Park Ridge, 60068 (Redeemer Lutheran Church Gym). Injury and Liability Release Form
In consideration for letting your child participate at Chicago’s Finest with Emily Thomas
I hereby release Deon and Emily Thomas and Redeemer Lutheran Church of Park Ridge, its trustees, employees, agents and representatives from any and all liability and agree to not raise any claims or institute any legal action against the Thomas' or Thomas Training or Chicago’s Finest, its trustees, employees, agents or representatives based upon any cause of action in my favor that arises out of or in connection with my child's participation in Sunday Skill Sessions with the Thomas'. This release shall apply to any loss of or damage to my property, and to any personal injury (including death) that I suffer, including, without being limited to, any loss, damage or injury sustained or allegedly sustained by me due to the negligent acts or omissions of the trustees, employees, agents or representatives of Thomas Training and Chicago’s Finest.
I fully understand and assume all of the risks, dangers and responsibilities connected with basketball and Chciago’s Finest at Redeemer Lutheran Church in Park Ridge.
